Underbite Cosmetic Reconstruction 
There are some cases that do not meet normal criteria for veneer placement. “Hard bites” such as found with people that have Class III occlusions (underbites) normally can be difficult and even impossible to improve esthetically due to the heavy demands created by the lower jaw upon the upper teeth.

For example, this young professional came to our office after consulting with every leading cosmetic expert from coast to coast. He had given up hope trying to find a solution to improving on what he called “a crooked and deficient smile.”  Nobody could provide a solution for improving his smile due to the extent of his underbite.

After careful analysis we came up with a game plan to improve his bite and make adequate room to place 8 Lumineers in order to achieve a more harmonious smile.

The following photos show the results of our treatment.  Notice the overall improvement in the facial appearance.

Patient was told nothing could be done!
Due to previous trauma, our young business executive had “put up” with yellow front teeth for many years.  His old dentist told him that nothing was possible for his teeth besides bleaching.  After careful x-ray examination we found that his front teeth were severely damaged and showed infections in the roots. 

Many teeth that suffer blows or even minor trauma can undergo slow degeneration and chronic low-grade infections that are hard to
detect.  It is important that proper x-rays are developed to ascertain the status of the health of these teeth.  Often, normal examinations
can miss such problems.

Root canal therapy was performed to stop the progress of the root breakdown and the teeth were reinforced with titanium posts. 
BriteSmile teeth whitening procedure was also completed in order to provide a more radiant smile.

Soon after, 8 Lumineers were fabricated and cemented on his front teeth.  The patient stated:  “All of these years went by and nobody
told me that this could be done for my teeth.  I am amazed to look at myself in the mirror now!  I can’t stop smilling!”

CEREC uses CAD/CAM (Computer Aided Design/Computer Aided Manufacturing) technology, incorporating a camera, computer and milling machine in one instrument.  The dentist uses a special camera to take an accurate picture of the damaged tooth. This optical impression is transferred and displayed on a color computer screen, where the dentist uses CAD technology to design the restoration.  Then CAM takes over and automatically creates the restoration while the patient waits. Finally, the dentist bonds the new restoration to the surface of the old tooth.

The whole process takes about one hour!

What does this innovation mean for a patient?

A tooth-colored restoration means no more silver fillings discoloring smiles. The filling is natural-looking, compatible with tissue in the mouth, anti-abrasive and plaque-resistant. Dentists no longer need to create temporaries or take impressions and send them to a lab. Because of this, the traditional second visit has been eliminated. CEREC 3D One-visit dentistry

CEREC^® is a high-tech device used to design and manufacture ceramic tooth restorations at the dental practice while the patient waits.  The CEREC equipment comprises a digital infrared camera for obtaining an optical impression of the damaged tooth, 3D Computer Aided Design (CAD) software to help the dentist design the ideal restoration for the clinical situation, and Computer Aided Manufacturing (CAM) technology that uses two high-speed diamond burs to mill a precise restoration according to the CAD software specifications.

The milling process is complete in less than 20 minutes, and the entire restorative procedure is completed in a single appointment.

Total Mouth Reconstruction
As tooth structures wear down with age, the physical properties of our bite also change. Left untreated for an extended time, the bite collapses (a natural aging phenomenon.)

As a result of this “aging process” patients can experience TMJ pain, increased wear on front teeth, changes in facial features, difficulty smiling, and can even cause us to look older than our real age (granny mouth).

Our patient, an attractive 30 something year old male had the beginning stages of dental problems that typically don't occur, as a natural phenomenon, until a person is in their late forties or fifties.

As can be seen in the above photographs, our patient had an extremely gummy smile, small upper teeth, and severe wear to the biting surfaces.

Our patient had several goals that needed to be attained:

1.      Fix the gummy smile

2.      Halt and reverse the wear and tear

Many other dentists had given this young man their opinions, which included "all at once" bite adjustments and cosmetic reconstruction.  We helped him understand that occlusion/bite modifications for his jaw structures needed to be changed incrementally, through a step by step process to assure natural repositioning of the upper and lower jaws.  Corrections made too quickly usually result in worsening jaw relationships.


Best Treatment Outline

Selective gum shaping and recontouring was used as needed to create harmony across the new gingival arches on the treated teeth.Davinci Veneers were used selectively to resize the anterior teeth, to the cosmetic specifications made by the client. The anterior resizing required the need for onlays and inlays on the posterior teeth…the bite was going to be in the beginning stages of "re-opening." Interestingly, our treatment cost was less than half of what other doctors recommended.

Pain Free Veneer Laminates!

The fitting process is essentially free of pain.  Traditional porcelain veneers usually require that the existing teeth are ground down to allow placement of the veneer.  The use of the Lumineers frequently eliminates the need for removing tooth structure.

For many patients there is no drilling, grinding or preparation that would require the usual need for a local anesthetic.  Also, because Lumineers are the thinnest porcelain laminate materials available today, there is no risk of having veneers that make your teeth appear "fat" or unusually "thick" (a common complaint with veneer makeovers).

Diastema Treatment

Smile Line Correction Perfection

Along with the popularity of cosmetic dentistry patients are learning more about the Golden Rule of Proportion,  an integral part
of cosmetic design that has become popularized by the Las Vegas Institute (LVI), a renowned cosmetic dentistry training facility for aesthetic and reconstructive dentists.

This concept relates to several physical properties of tooth structures, especially those that appear when we smile broadly.  Accomplished makeover dentists typically use several porcelain veneers that are handcrafted for each tooth structure.

Certain steps are followed to create a perfect smile that has all of the upper anterior tooth structures
in perfect harmony.  Each tooth is individually resized and reshaped with the new ceramic material.

This style of makeover dentistry works very well for people who need several veneers (usually 6 to 8 as the minimum).  Many dentists won't attempt veneer makeovers with less than 6 veneers. Some dentists are known to require 10 veneers for this kind of cosmetic result.


Our patient had a very specific goal of having a “near perfect” smile that matched her face and looked
natural. She had visited several local area dentists.  Despite having great looking, healthy teeth, all of the dentists recommended several veneers that would have require significant tooth preparation to achieve the result she wanted.  The patient did not feel comfortable with these recommendations.

Dr. Halmaghi developed a diagnostic waxup of the patient's upper arch and was able to demonstrate the use of some minor tooth reshaping along with the use of only 2 veneer products that would
require no invasive preparation.

Lumineers™ by Cerinate provided the treatment solution that would create the best aesthetic outcome possible for the patient.

Comparing the 1st before photo with the 2nd after photo it can be seen that resizing the patient's lateral incisors (teeth on each side of the two large front teeth) in width and height provided the appearance she was seeking.

The final cosmetic result was quite astounding, given that multiple veneers were not necessary. 
Dr. Halmaghi was able to create the smile she wanted at a mere fraction of the cost expected... and most importantly, with no invasive tooth preparation.

Causes of Tooth Resorption

Genetics, developmental abnormalities, diet, complications of physical disease and/or treatments, medications and trauma are known to start the resorption process.

Cosmetic and reconstructive treatments for this condition are varied and determined primarily by certain causal factors, predictions for long term success and the cosmetic and functional goals wanted by the patient.

Trauma Induced Dental Resorption

Our first photo reflects resorption of an anterior tooth that was originally initiated by physical trauma. In addition to the missing tooth, resorption and gum recession can be readily seen on the affected anterior tooth.

The patient, a young female, had become very self conscious about her appearance and, more importantly, was concerned about her long term oral health.

Tooth Resorption - Treatment Plan Choices

During the consultation Dr. Halmaghi presented a varied choice of treatment options, each with different levels of cosmetic result, dental function, long term predictability for success and affordability concerns.

Prior to coming to our office, the patient had already met with several dentists in the Soutfield - Detroit area who were unable to deliver the results she was seeking.

Our patient elected to use a combination of premium grade porcelain veneers and custom bridgework to create the cosmetic and functional outcome she was seeking that would be the most natural in appearance and feel.

Old bonding and veneer products were removed and replaced with her choice of porcelain laminates that created the smile result she was looking for. Dr. Halmaghi was careful to assure that the veneer products and her bridgework matched perfectly.

As can be seen from our post treatment photo... the outcome was very successful and exceeded the patient's expectations.

The above photos show the combination of Fortress veneers and Mirage bridge porcelain used in this case. Two very unique porcelain systems were used to create balance between these teeth.

Many times when veneers and bridges are done together the final color match can be hard to achieve. The various systems of veneers that we can use today allow us to create the most lifelike looking teeth.

The missing tooth was replaced with a bonded Mirage bridge, reinforced by precious metal.

When we cemented ("glued") these teeth permanently we paid careful attention to the color of the cement and the technique used to make sure that no changes in the color would be seen immediately, or after a few years. Stability is the key to making this dental work last a lifetime.

The results are quite spectacular, as can been seen in the very alluring after photo. After her dental reconstruction, the patient's personal outlook and personality underwent significant change.... becoming more outgoing and smiling all day long. She even changed her hairdresser!
