Treatment for Over Crowed Teeth
This patient had been coming to our office for over 10 years and presented to us an obvious desire to have his orthodontic needs
treated. He had received orthodontic evaluation from several other offices with recommendations for premolar extraction to create space for orthodontic movement. Due to the severe extent of the crowding (teeth too large for his jaws), it was necessary to perform extraction of upper second molars and lower premolars.
This extensive treatment was completed in 18 months. Yes, 18 months from start to finish!
A series of heat activated wires and functional appliances were used to finish this case in such a short time frame. Notice the beautiful round smile line that was achieved utilizing extraction of the upper second molars, instead of the premolar teeth (which is still standard of treatment today in most orthodontic offices and
which was recommended for him by other orthodontists.)
Proper Developing Esthetics
Many cases that are treated using maxillary (upper jaw) premolar extraction end up leaving the patient with a smaller and narrower arch form (shape of the jaw) due to the fact that upper premolar extraction causes “shrinking” of the upper jaw.” Many times this can be observed by noticing that such results leave the patient with a longer nose and smaller cheeks, as the face tends to take on the “sunken-in” appearance, especially when viewed from the side.
Second molar upper extractions actually provide additional forces for jaw growth and proper developing esthetics, by diminishing the negative effects of crowding and allowing the jaw bones to “relax” into proper growth.
